Referrals

If your practitioner suggests a referral to a specialist, we will make the appointment and call you with the date and time. If you make your own appointment, please call Jann at 330-684-2015 (select option 2) with the appointment date and time. She is available Monday through Friday from 7:30 am – 3:00 pm. Please allow 2 business days for the referral to be authorized; less urgent referrals may take longer.
