Medication Refills

Before calling, please check with your pharmacy
to see if there are refills available

If a prescription has been refilled do not give the pharmacy the old prescription number. Tell them you have a new prescription.

Please have ready:

•	Spelling of your first and last name
•	Pharmacy name and number
•	Spelling of your medication name
•	The phone number and time at which to reach you if we have any questions
•	Tell us whether you want to pick up a written prescription or if we should call the prescription to the pharmacy

There will be a charge for duplicate prescription refill requests (i.e. lost prescriptions, changes to insurance plans, etc.).

If you are requesting a prescription and have not been seen for over a year you will be asked to schedule an appointment to see your practitioner.

Please plan ahead. 3 business days notice required on all refills.
No medications will be refilled after 4:00 pm or on Saturdays.
